Kids everywhere are playing the Pokémon Trading Card Game. We are excited that you are interested in Pokémon Organized Play. To get started, please print out this permission form and have your parent or guardian fill it out. Then send it to the address or fax number provided on the form.

Please ensure your browser is set to print on letter size paper; The permission form will not print correctly on any other size paper.
